Mouth Ulcer
Most mouth ulcers are harmless and heal within two weeks. But an ulcer that persists beyond 3 weeks, is painless, or has hardened edges requires specialist evaluation to rule out oral cancer — the most important diagnosis not to miss.
When a Mouth Ulcer Needs Urgent Evaluation
- Ulcer present for more than 3 weeks
- Painless ulcer (malignant ulcers are often painless)
- Hardened (indurated) edges or base
- Associated lump or swelling in the jaw or neck
- Difficulty swallowing or opening the mouth
- Numbness of the lip, tongue, or chin
- Unexplained bleeding from the ulcer
- Associated white or red patch around the ulcer
Types of Mouth Ulcer
Aphthous Ulcer (Canker Sore)
BenignThe most common type. Small, round, painful ulcers with a white or yellow centre and red halo. Usually heal in 7–14 days without treatment. Often recurrent. Not caused by infection and not contagious.
Traumatic Ulcer
BenignCaused by a sharp tooth, ill-fitting denture, or accidental cheek bite. Heals once the cause is removed — usually within 2 weeks. If it does not heal after removing the cause, specialist evaluation is needed.
Leukoplakia / Erythroplakia
PremalignantWhite or red patches (not true ulcers) that can become ulcerated. These are premalignant conditions — some progress to oral cancer. A biopsy is required for all persistent white or red patches.
Oral Cancer (Carcinoma)
MalignantA non-healing ulcer that persists beyond 3 weeks, particularly with hardened edges, pain on pressure, and does not respond to standard treatment. Biopsy is mandatory to confirm or exclude malignancy.
Viral Ulcer (Herpetic)
BenignCaused by Herpes simplex virus. Multiple small vesicles that burst to form shallow ulcers, often on the lips (cold sores) or palate. Usually heal in 10–14 days. Antiviral treatment shortens duration.
Oral Lichen Planus
MonitorAn immune-mediated condition causing lacy white patches (Wickham striae) or painful erosive ulcers. Requires regular specialist review due to a small risk of malignant transformation over time.
